1.進(jìn)口復(fù)出口的：
（1）提交由藥品監(jiān)督管理部門批準(zhǔn)出具的進(jìn)口醫(yī)療注冊(cè)證明。
（2）提交出口醫(yī)療物資聲明【附件1】。
(1) Submit the import medical registration certificate approved and issued by the drug regulatory department.
(2) Submit the declaration of exported medical materials [Annex 1].
2.未獲得國內(nèi)醫(yī)療器械產(chǎn)品注冊(cè)證書，但獲得國外標(biāo)準(zhǔn)認(rèn)證或注冊(cè)的醫(yī)療物資:
（1）提交出口醫(yī)療物資聲明（中英文）【附件2】；
（2）生產(chǎn)企業(yè)需在商務(wù)部提供的取得國外標(biāo)準(zhǔn)認(rèn)證或注冊(cè)的生產(chǎn)企業(yè)清單中。（中國醫(yī)藥保健品進(jìn)出口商會(huì)網(wǎng)站動(dòng)態(tài)更新）
(1) Submit the statement of export medical materials (in Chinese and English) [Annex 2];
(2) Production enterprises shall be listed in the list of production enterprises with foreign standard certification or registration provided by the Ministry of Commerce. (website update of China Chamber of Commerce for import and export of medical and health products)
